Filbert Street <br /> Stockton, CA 95205 <br /> Dear Mr. Lopez, <br /> The purpose of this letter is to respond to the Inspection Report received by their location in Durham on <br /> May 24, 2018. <br /> Item#4 Failed to complete and/or submit the Business Activities or Owner/Operator Identification page <br /> Item#5 Failed to complete and/or submit hazardous materials inventory for all reportable materials on <br /> site <br /> Item#6 Failed to complete and/or submit a site map with all required content. <br /> Item#12Failed to have business plan readily available to facility or EHD personnel. <br /> National Express did not have lead user rights to update the required information in CERS. Upon <br /> receiving access to the database, information was updated and submitted to your Agency for <br /> review/approval. CERS information was submitted to your Agency for review on June 25, 2018. <br /> Item#7 Emergency response procedures were lacking or inadequate <br /> National Express updated the Emergency Response and Training Plans included within the CERS <br /> database and submitted to your Agency for review/approval on June 25, 2018. <br /> Item#10 Failed to submit revisions to business plans within 30 days of the substantial change in <br /> operations <br /> National Express did not have lead user rights to update the required information in CERS. Upon <br /> receiving access to the database, information was updated and submitted to your Agency for <br /> review/approval.
