<br /> The following data elements have not been completed adequately:
<br /> A. Business Owner/Operator Identification Page:
<br /> 1 . Side 1 - Complete/Correct #10 and #40 .
<br /> 2 . Side 2 - Complete/Correct #57 .
<br /> B. Hazardous Materials Management Plan:
<br /> 1 . Side 1 - Complete/Correct the Business Name at the top of
<br /> the page and Data Elements #6, #9, #10, and #11 .
<br /> 2 . Side 2 - Complete/Correct #14 - Note, employees must, at
<br /> a minimum, be authorized to safely isolate the area and
<br /> make notifications in the event of an actual or
<br /> threatened release.
<br /> C. Facility Map:
<br /> 1. Your facility map must show the following information:
<br /> Adjacent property use, parking lots, storm drains, sewer
<br /> drains, spill control equipment.
<br /> 2 . The Sub-Site Maps must be clearly named with the portion
<br /> of your facility being shown. The name must include the
<br /> grid coordinates of the Primary Site Map which are
<br /> covered by the Sub-Site Map, e.g. Primary Production
<br /> Building (D-2) . "D-2" refers to the portion of the
<br /> Primary Site Map where the Primary Production Building is
<br /> located. The Sub-Site Maps must show all required
<br /> information that is not shown on the Primary Site Map.
<br /> D. Chemical Description Page:
<br /> 1 . LPG, correct/complete #6, #7, #14, #17, #18, #20, #21,
<br /> #27, #28, and #33 .
<br /> 2 . Oxygen, correct/complete #6, #7, #9, #18, #28, #33 , and
<br /> #34 .
<br /> 3 . Acetylene, correct/complete #6, #7, #15, #18, #28, #33 ,
<br /> and #34 .
<br /> 4 . Waste Oil, correct/complete #5, #6, #7, #14, #17, #18,
<br /> #19, #20, #21, #22, #23 , #24, #27, #28, and #33 .
