The following is an itemized list of hazardous materials violations that have not been <br /> addressed for VALLEY FORKLIFT as of November26, 2018. <br /> Open violations from October 09, 2018 inspection <br /> Violation#5 - Failed to complete and/or submit hazardous materials inventory for all reportable materials on <br /> site. <br /> Four 96 gallon forklift batteries, one 55 gallon drum of diesel exhaust fluid, one 100 gallon tank of waste antifreeze, <br /> one 500 gallon propane tank, and at least 103 gallons of new motor oil were found on site that have not been <br /> reported. Any material that meets or exceeds the reportable quantity shall be reported in the facility's business plan. <br /> Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, add the <br /> materials to the hazardous material inventory, and submit to the EHD for approval. <br /> Violation#6 - Failed to complete and/or submit a site map with all required content. <br /> The maps do not show loading areas, emergency shutoffs, hazardous material handling and storage areas <br /> (compressed gasses, waste antifreeze, diesel exhaust fluid, motor oil, and the lead acid batteries), emergency <br /> response equipment(spill control); map 2 does not contain a north orientation. A site map shall contain a north <br /> orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, access and exit points, <br /> emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, and emergency <br /> response equipment. If a site map element is not applicable for your facility then list it on the map and label as <br /> "NA". Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, <br /> upload the correct or updated information, and submit to the EHD for approval. <br /> Violation#7 - Failed to establish and/or submit emergency response procedures for a release or threatened <br /> release. <br /> The emergency response procedures were lacking. Section C. the phone number for the Regional Water Quality <br /> Control Board, 916-464-3291 has not been identified; Section G. Location has not been completed for some of the <br /> emergency response equipment. A business plan shall include the following emergency response procedures for a <br /> release or threatened release of hazardous materials, including, but not limited to, the following: <br /> - Immediate notification of local emergency personnel and the Environmental Health Department(EHD) <br /> - Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment <br /> - Evacuation plans and procedures, including immediate notice, for the site <br /> Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the <br /> correct or updated information, and submit to the EHD for approval. <br /> Violation #9 - Failed to annually review and certify that the business plan is complete, accurate, and up to <br /> date. <br /> The business plan information has not been reviewed and resubmitted in the California Environmental Reporting <br /> System (CERS) annually. The hazardous materials inventory shall be submitted by January 15 of each calendar <br /> year and may be submitted beginning November 1 of the previous year.
