Your facility has been assigned the Business Account Number <br /> of 10482. Please be sure to place this account number in the appropriate field in template forms. <br /> To install the template files,download the "SETUP" File attached to this e-mail message onto your hard <br /> drive. Create an empty "templates" folder in your hard drive to contain the files that will be installed on your <br /> computer. Then open the "SETUP" File and follow the installation instructions as you would with any new <br /> software. The program should create a "shortcut" icon on your desktop which you can use to open the <br /> templates. If it fails to do so,the file named "HMMP97Solution" should be opened first from your <br /> "templates" folder. After installing the template files you can discard the original "Setup" file. <br /> Instructions for completing the templates and for transmitting electronic copies back to us are contained in the <br /> program and can be accessed from the Main Menu. <br /> Please transmit your completed templates to us by disk or e-mail. Do not print and transmit hard <br /> copies. Send the facility map(s)to us by hard copy or if you have the capability to scan your map(s) send a <br /> copy of the digitized map file in JPEG, TIF,or GIF format along with the template files. <br /> If you submit your templates electronically you do not have to submit the Declaration of Completeness and <br /> Accuracy Form. Do print hard copies of theIan for use by your employees. <br /> DUE DATE: The HMMP plan for your facility must be submitted by October 10, 2001. <br /> Please send back a reply to this e-mail message confirming that you have received the templates.
