j !1 The following is an itemized list of hazardous materials violations that have not been addressed for HEADWATERS RESOURCES INC as of March 25, 2016. Open violations from January 26, 2016 inspection Violation#3 -Failed to annually review and certify that the business plan is complete,accurate,and up to date. The business plan information has not been reviewed and resubmitted in the California Environmental Reporting System (CERS)annually. The hazardous materials inventory shall be submitted by January 15 of each calendar yes and may be submitted beginning November 1 of the previous year. Immediately log into the CERS at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. Violation#6-Failed to complete and/or submit the Business Activities or Owner/Operator Identification page. The Business Owner/Operator Identification Form and the Business Activities form were not submitted for the years 2013, 2014, 2015 and 2016. This information must be current and submitted as part of the business plan. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. Violation#11 -Failed to establish and/or submit emergency response procedures for a release or threatened release. The emergency response procedures have not been submitted for the years 2013, 2014, 2015 and 2016. A busines plan shall include the following emergency response procedures for a release or threatened release of hazardous materials, including, but not limited to, the following: -immediate notification of local emergency personnel and the Environmental Health Department(EHD) -procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to persons, property, or the environment -evacuation plans and procedures, including immediate notice, for the site Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. Violation#13-Failed to provide or document initial and/or refresher training to appropriate personnel. Proof of relevant employee training was not available at the time of inspection. The business plan shall include provisions for ensuring that appropriate personnel receive initial and annual refresher training. This training shall be documented electronically or by hard copy and shall be made available for a minimum of three years.
