Blank 1999 Chemical Description Page Form <br /> 3. Program Newsletter <br /> 4. Note on Federal Inventory Reporting Requirements <br /> Please review your HMMP and chemical inventory and make necessary revisions. You are required <br /> to retain a current and accurate copy of your HMMP and chemical inventory at your facility. <br /> If you are preparing your HMMP and inventory on a computer with our electronic templates, you <br /> must submit initial/updated forms electronically. Please make necessary changes on your computer <br /> and send a complete electronic copy of the HMMP97.FP, CHEM97.FP3, and if appropriate, <br /> STAFF97.FP3 files to us by disk, First Class E-mail, or Internet (xsjoes2@pacbell.net). If you • <br /> send in a disk, please sign and enclose the Certification Statement. If you use E-mail, send the <br /> original of your Certification Statement by mail. . <br /> If you have not been maintaining and updating your HMMP electronically and are now interested in <br /> using this simpler means of compliance,please call our office. We can provide form templates and <br /> instructions on disks or over the Internet. You only need Windows95 or a Macintosh Power PC. <br /> If you have any questions, please contact our office at(209)468-3969. The Certification Statement, <br /> with any necessary revised hard copy forms or revised electronic copies of template files are due <br /> January 15, 1999.
