The current information is incomplete. <br /> You must correct the information entered in Data Elements #5 <br /> (Describe where at your facility the CO, is located) , #27, and #28 <br /> on the Chemical Description Page for Carbon Dioxide. <br /> Your facility map fails to show the following topographical <br /> information: adjacent property use, parking lots, and employee <br /> evacuation assembly area. <br /> Your facility map fails to show the location(s) of the following <br /> safety equipment: fire extinguishers, first aid supplies, fire <br /> alarms,and shut off valves. <br /> You must enter your business name and an approximate scale on your <br /> facility maps. <br />
