The emergency response procedures were not complete. The local Unified Program Agency(UPA)phone number on line C3 is not correct. Please list 209-468-3420 as the local UPA phone number. A business plan shall include the following emergency response procedures for a release or threatened release of hazardous materials, including, but not limited to,the following: -immediate notification of local emergency personnel and the Environmental Health Department(EHD) -procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to persons, property, or the environment -evacuation plans and procedures, including immediate notice,for the site Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. This is a Class II violation. 9 HSC 25508(a)(1) Failed to annually review and certify that the business plan is complete, accurate, and up to date. The business plan information has not been reviewed and resubmitted in the California Environmental Reporting System (CERS)annually.The last submittal was not made between 11/17/2017- 1/15/2018. The last submittal was made on 1/16/2018 making the submittal late. This violation has been corrected on site. The hazardous materials inventory shall be submitted by January 15 of each calendar year and may be submitted beginning November 1 of the previous year. Immediately log into the CERS at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. This is a Class II violation. 15 HSC 25510(a)Failed to report actual or threatened release to the EHD and/or CalOES Warning Center. A release of diesel occurred on or before 12/18/2018 and was not reported to the EHD and CalOES. Dark staining, absorbent like material on ground, and a strong oder of diesel was observed during the inspection. A release or threatened release of a hazardous material shall be verbally reported immediately to the EHD and CalOES. Ensure that any future releases or threatened releases of a hazardous material are verbally reported immediately to the EHD and CalOES. This is a Class I violation. Overall Inspection Comments: Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement documenting the corrective actions that have been or will be taken for each violation, and any supporting paperwork, by 1/17/2019. This inspection was supposed to be a closing inspection. The diesel in the generator and the propane on site is keeping this business from being able to be inactivated from the Hazardous Material Business Plan program. Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate ($152).
