OAQ U I I1 <br />COUNTY— <br />Environmental Health Department <br />Hazardous Materials Program Inspection Report <br />Facility Name: <br />Facility Address: <br />Date: <br />TEREX UTILITIES INC <br />2610 LYCOMING ST, STOCKTON <br />September 11, 2018 <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR- Notice to Comply) <br />Item # <br />Remarks <br />6 <br />HSC 25505(a)(2) Failed to complete and/or submit a site map with all required content. <br />The map(s) did not show loading areas, internal roads, emergency shutoffs, emergency response equipment (spill <br />control equipment), the legend does not identify all of the codes used on the maps, and the map have not been <br />updated to remove the bulk chemical storage area. A site map shall contain a north orientation, loading areas, <br />internal roads, adjacent streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation <br />staging areas, hazardous material handling and storage areas, and emergency response equipment. If a site map <br />element is not applicable for your facility then list it on the map and label as "NA". Immediately log into the California <br />Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the correct or updated information, <br />and submit to the EHD for approval. <br />This is a minor violation. <br />7 <br />HSC 25505(a)(3) Failed to establish and/or submit emergency response procedures for a release or threatened <br />release. <br />The emergency response procedures were lacking. Section C. the phone numbers for the Local Unified Program <br />Agency, 209-468-3420 and Section J. has not been completed. A business plan shall include the following <br />emergency response procedures for a release or threatened release of hazardous materials, including, but not <br />limited to, the following: <br />- Immediate notification of local emergency personnel and the Environmental Health Department (EHD) <br />- Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br />persons, property, or the environment <br />- Evacuation plans and procedures, including immediate notice, for the site <br />Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the <br />correct or updated information, and submit to the EHD for approval. <br />This is a Class II violation. <br />9 <br />HSC 25508(a)(1) Failed to annually review and certify that the business plan is complete, accurate, and up to date. <br />The business plan information has not been reviewed and resubmitted in the California Environmental Reporting <br />System (CERS) annually. The hazardous materials inventory shall be submitted by January 15 of each calendar <br />year and may be submitted beginning November 1 of the previous year.
