Program Newsletter <br /> 2. 2002 Certification Statement with instructions <br /> 3. Blank 2002 Chemical Description Page Form <br /> 4. Note on Federal Inventory Reporting Requirements <br /> 5. Electronic Compliance Process Questionnaire <br /> Please review your HMMP and chemical inventory, make necessary revisions, and submit to our <br /> office with the enclosed Certification Statement. You are required to retain a current and accurate <br /> copy of your HMMP,chemical inventory, and latest Certification Statement at your facility. <br /> If you are preparing your HMMP and inventory on a computer with our electronic templates, you <br /> must submit initial or updated forms electronically since this keeps our costs and <br /> your fees as low as possible. Please make necessary changes on your computer and send a <br /> complete electronic copy of the HMMP97, CHEM97, and if appropriate, STAFF97 files to us by <br /> disk or Internet (xsjoes@co.sanjoaquin.ca.us). If you send in a disk,please sign and enclose the <br /> Certification Statement. If you use E-mail, send your signed original Certification Statement by <br /> mail. <br /> If you have not been maintaining and updating your HMMP electronically and are now interested <br /> in using this compliance tool,please call our office. We can provide templates and instructions to <br /> you by disk or e-mail. <br /> If you have any questions, please contact our office at(209)468-3969. The Certification <br /> Statement, with any necessary revised hard copy forms or electronic copies of template files are <br /> due on Thursday, January 17, 2002.
