UNIVAR USA INC. <br />40-4, univAR <br />CONTINGENCY PLAN <br />06/14/11 Replaces 03/10/10 <br />SECTION <br />SUBJECT PAGE 21 <br />SIX <br />EMERGENCY PROCEDURES OF 22 <br />HANDLING A CALL FROM CHEMTREC or INFOTRAC <br />CHEMTREC - Univar USA utilizes the CHEMTREC phone number for our 24/7 <br />emergency phone number on all bills of lading; not to be utilized for <br />waste shipments. <br />INFOTRAC - Univar USA utilizes the INFOTRAC phone number for our 24/7 <br />emergency phone number on all hazardous and non-hazardous waste <br />shipments; this includes ChemCare transactions. <br />Administrative Issues <br />Phone numbers for Emergency Coordinators and Alternates are filed and maintained with <br />CHEMTREC and lnfotrac. These files are maintained by Redmond Operations; updates are <br />sent out to both CHEMTREC and lnfotrac on a quarterly basis. Branch Operations <br />Managers/Supervisors should report changes as soon as known to Redmond Operations. <br />CHEMTREC and lnfotrac personnel have been instructed to contact the Emergency <br />Coordinator located closest to where the incident is occurring. However, in the event the <br />closest Emergency Coordinator is unavailable, CHEMTREC or Infotrac personnel will contact <br />the next logical (closest) Emergency Coordinator(s). <br />Univar USA Inc. is registered with CHEMTREC and Infotrac, enabling Univar USA to use <br />CHEMTREC's phone number (1-800-424-9300) as the 24-hour emergency response <br />number on bills of lading and Infotrac's phone number (1-800-535-5053) on manifests. <br />Protocol <br />Incident Involves Emergency Coordinator's Customer/Carrier/ Shipment <br />When CHEMTREC or lnfotrac personnel contact a Univar USA Emergency Coordinator or <br />alternate, that individual will: <br />Obtain known information about the event from the CHEMTREC or lnfotrac <br />personnel. <br />Assume responsibility for action from CHEMTREC or lnfotrac. <br />Establish telecommunications with the "caller" to CHEMTREC or Infotrac as <br />soon as possible. <br />Learn more details about the incident and provide the "caller" safety information <br />found in Emergency Response Guides and/or MSDS's. <br />Implement the Contingency Plan, if appropriate.
