UNIVAR USA INC. <br /> 0 u n IN/AR <br />CONTINGENCY PLAN <br />')6/14/11 Replaces 03/10/10 <br />SECTION <br />SUBJECT PAGE 1 <br />TWELVE <br /> <br />REPORTING/CRITIQUE OF 2 <br />REPORTING <br />The Emergency Coordinator must report the emergency as soon as possible after the <br />emergency has been resolved. <br />Contact the Regional Regulatory Manager or Regional Operations Manager as <br />soon as possible (immediately). <br />The Regional Regulatory Manager (or Regional Operations Manager if the <br />Regional Regulatory Manager is not available) will notify the appropriate State <br />and Federal regulatory authorities of the incident. Senior Management <br />notification will normally be made by Corporate Operations in Redmond. <br />Complete an Incident Notification Report and e-mail the notification to the <br />Regional Regulatory Manager and Operations Reports. <br />Required written reports will be submitted by the Regional Regulatory Manager in <br />conjunction with the Emergency Coordinator and Corporate Operations in <br />Redmond. <br />If reportable quantities of hazardous materials are released, the report of the <br />release will be made to the National Response Center and the proper State and <br />local authorities by the Regional Regulatory Manager. <br />The required notification to local, State, and Federal regulatory authorities that <br />the branch has returned to normal operations and is in compliance with all <br />permit requirements must be made before operations resume. This notification <br />will be made by the Regional Regulatory Manager in conjunction with the <br />Emergency Coordinator. <br />7 A report on the implementation of the Contingency Plan, if the plan is <br />implemented for an emergency involving hazardous wastes, must be submitted to <br />the appropriate Federal and State regulatory agencies. The report will be <br />submitted by the Regional Regulatory Manager in conjunction with the <br />Emergency Coordinator. The format and content of the report will be that <br />required by the regulations.
