SAN JOAQUIN COUNTY <br />OFFICE OF EMERGENCY SERVICES <br />HAZARDOUS MATERIALS PROGRAM <br />Request for Corrective Action <br />Account No: 14720 <br />The Corrective Actions below must be completed by April 12, 2012 <br />In Reference to this matter, please ask forRobert Lopez <br />CORRECTIVE ACTIONS NEEDED <br />The Business Plan/Chemical Inventory have not been completed satisfactorily. <br />Consequently, it is necessary for you or a representative of your <br />company/organization to come into the office to receive assistance in completing the <br />corrections required for the Business Plan to be accepted. <br />Specifically, the recent map submittal does not conform to the requirements specified <br />in the instructions for completing and submitting the facility map. The instructions are <br />available on the Compliance Hazardous Materials Management Program <br />Compliance Website at the link "Preparing And Submitting Facility Map (s)". Note, <br />the map must be completed on the required map form. <br />As a courtesy, an appointment to complete the necessary corrections has been <br />scheduled for April 12, 2012 at 8:30 A.M. Please, call to confirm that you or a <br />representative of your company/organization will be able to attend at the scheduled <br />date and time or if you need to reschedule.
