I did create a work request on 1/10/12 to close the account #12983 and to create a new account for Univar USA, Inc. So <br />far the account #12983 has been closed so you do not have to address the Annual Certification. Under normal circumstances you would have <br />already received a you new account information, but, due large number of businesses that are required to comply with the Annual Certification <br />deadline, it has created a back log. With that said, I will check tomorrow and see if I can not expedite the process. <br />Below is a copy of a portion of the work request I created. Please, check the contact information a verify if it is correct. <br />I • Univar USA, Inc. <br />I. Create a new Y/Y account and send a new business letter due in 30 days to: <br />Contact: Brian L.
