The primary and secondary emergency <br />contacts are no longer with the business . Please update the primary and secondary emergency contact names. <br />This information must be current and submitted as part of the business plan. <br />This violation was corrected on site. <br />This is a minor violation. <br /> 5 HSC 25506(a)(1) Failed to complete and/or submit hazardous materials inventory for all reportable materials on site. <br />150 gallons of used anti freeze, 250 cubic feet of acetylene, and 500 gallons of propane were found on site that <br />have not been reported. Any material that meets or exceeds the reportable quantity shall be reported in the facility's <br />business plan. This violation was corrected on site. <br />This is a minor violation. <br /> 6 HSC 25505(a)(2) Failed to complete and/or submit a site map with all required content. <br />The site map was not complete as part of the business plan. The site map is missing emergency response <br />equipment locations (fire extinguishers and spill kits), exits and accesses points, loading unloading area, utilities <br />shut offs, and an evacuation staging area. <br />Facility does not have emergency shut offs and storm drains, these elements were not addressed on the site map. <br />Please address this by either writing “NA” for not applicable or a statement that says the element is not on site. <br />Please correct this by the next submittal <br /> A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer <br />drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and <br />storage areas, and emergency response equipment. If a site map element is not applicable for your facility then list <br />it on the map and label as “NA". Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br />This is a minor violation. <br /> 9 HSC 25508(a)(1) Failed to annually review and certify that the business plan is complete, accurate, and up to date. <br />The business plan information has not been reviewed and resubmitted in the California Environmental Reporting <br />System (CERS) annually. The last submittal made was 1/15/2015. Please make a submittal for 2018. <br /> The hazardous materials inventory shall be submitted by January 15 of each calendar year and may be submitted <br />beginning November 1 of the previous year.
