This can be accomplished through our web site. <br /> Going on to the web site and entering your username and password you will enter the main menu area. <br /> Scrolling down and looking on the right side you will see "Annual Certification Form". Go to the second bullet <br /> "Office of Emergency Services Certification Form". Click into this one and you will see the annual <br /> certification statement. Read the statement, check the box once and hit submit.
