Business Owner/Operator Identification Page: <br /> 1. Side 1 - Complete/Correct #10, and #11 . <br /> 2 . Side 2 - Complete/Correct #58 and #59 - training is required <br /> by law. Reference Title 19 California Code of Regulations, <br /> Chapter 4 Article 4 Section 2732 Training and the 1998 Uniform <br /> Fire Code, Appendix ll-E, Hazardous Materials Management Plan <br /> Section 3 .2 .8 Employee Training. <br /> A training program appropriate to the types and quantities of <br /> materials stored or used shall be conducted to prepare employees <br /> to safely handle hazardous materials on a daily basis and during <br /> emergencies . The Training program shall include: <br /> 1 . Instruction in safe storage and handling of hazardous <br /> materials, including maintenance of monitoring records, <br /> 2 . Instruction in emergency procedures for leaks, spills, fires <br /> or explosions, including shutdown of operations and evacuation <br /> procedures, and <br /> 3 . Record-keeping procedures for documenting training given to <br /> employees. <br /> B. Hazardous Materials Management Plan: <br /> 1 . Side 2 - Complete/Correct #14 - Note, employees must, at a <br /> minimum, be authorized to safely isolate the area and make <br /> notifications in the event of an actual or threatened release. <br /> C. Facility Map: <br /> 1. Your facility map must be submitted on the 8 . 5" x 11" map <br /> page grid that came with your HMMP package. <br /> 2 . Your facility map must show the following information: <br /> loading areas, internal roads, adjacent property use, access and <br /> egress roads, underground water systems or wells, parking lots, <br /> storm drains, sewer drains, flow of surface water, employee <br /> evacuation assembly area, facility entrances & exits, spill <br /> control equipment, emergency respirators, first aid supplies, <br />
