The following is an itemized list of hazardous materials violations that have not been <br />addressed for WESTERN TRUCK CENTER as of May25, 2017. <br />Open violations from April 05, 2017 inspection <br />Violation #5 - Failed to complete and/or submit hazardous materials inventory for all reportable materials on <br />site. <br />Reportable amount of waste flammable liquid (gasoline) was found on site that has not been reported. Any material <br />that meets or exceeds the reportable quantity shall be reported in the facility's business plan. Immediately log into <br />the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, add the materials to the <br />hazardous material inventory, and submit to the EHD for approval. <br />Violation #6 - Failed to complete and/or submit a site map with all required content. <br />One of the site maps did not include a legend/key to identify the code used to denote hazardous material storage <br />containers and none of the site maps show the hazardous material/waste storage in the wash bay. A site map shall <br />contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, access and exit <br />points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, and <br />emergency response equipment. Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br />Violation #11 - Failed to provide or document initial and/or refresher training to appropriate personnel. <br />Employee training records were not available at the time of inspection. The business plan shall include provisions <br />for ensuring that appropriate personnel receive initial and annual refresher training. This training shall be <br />documented electronically or by hard copy and shall be made available for a minimum of three years. Immediately <br />provide employee training for appropriate personnel and submit a copy of the training records to the EHD. A site <br />map was not completed or submitted as part of the business plan. <br />Violation #12 - Failed to have the business plan readily available to facility or EHD personnel. <br />Facility could not provide the Emergency Response Plan and the Chemical inventory upon request. The emergency <br />response plans and procedures, the inventory of information required by this article, and the site map required by <br />this section shall be readily available to personnel of the business or the unified program facility with responsibilities <br />for emergency response or training pursuant to this section. Immediately make arrangements to have the business <br />plan readily available for review. <br />Page 1 of 1 <br />
