A Hazardous Materials Business <br />Plan shall be established and implemented for emergency response to a release or threatened release of a <br />hazardous material when storing hazardous materials at or above the thresholds quantities of 55 gallons for <br />materials that are liquids, 500 pounds for solids, or 200 cubic feet for compressed gas, at standard temperature and <br />pressure. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, <br />enter the required information, and submit for review by the EHD. <br />This is a Class II violation. <br /> 4 HSC 25508(a)(1) Failed to complete and/or submit the Business Activities or Owner/Operator Identification page. <br />The Business Owner/Operator Identification form was not submitted or complete. The primary and secondary <br />contact information were no longer current. This information must be current and submitted as part of the business <br />plan. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, <br />enter the correct or updated information, and submit to the EHD for approval. Corrected. <br />This is a minor violation. <br /> 6 HSC 25505(a)(2) Failed to complete and/or submit a site map with all required content. <br />A site map was not completed or submitted as part of the business plan. A site map shall contain a north <br />orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, access and exit points, <br />emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, and emergency <br />response equipment. Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br />This is a minor violation. <br /> 8 HSC 25505(a)(4) Failed to submit a training program or program is not reasonable or adequate for facility. <br />Employees not trained and no records on hand. The business plan shall include provisions for ensuring that <br />appropriate personnel receive initial and annual refresher training. This training shall be documented electronically <br />or by hard copy and shall be made available for a minimum of three years.
