Environmental Health Department <br />Hazardous Materials Program Inspection Report <br />Date: <br />June 19, 2018 <br /> Facility Address: <br /> 8700 N THORNTON RD, STOCKTON <br /> Facility Name: <br /> BINH'S AUTO REPAIR <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br /> 6 HSC 25505(a)(2) Failed to complete and/or submit a site map with all required content. <br />A site map is missing location of used oil as part of the business plan. A site map shall contain a north orientation, <br />loading areas, internal roads, adjacent streets, storm and sewer drains, access and exit points, emergency shutoffs, <br />evacuation staging areas, hazardous material handling and storage areas, and emergency response equipment. If <br />a site map element is not applicable for your facility then list it on the map and label as “NA". Immediately log into <br />the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the correct or updated <br />information, and submit to the EHD for approval. <br />This is a minor violation. <br /> 7 HSC 25505(a)(3) Failed to establish and/or submit emergency response procedures for a release or threatened <br />release. <br />The emergency response procedures were not completed. SECTION E3, SECTION H LOCATIONS, SECTION H <br />LOCATIONS . A business plan shall include the following emergency response procedures for a release or <br />threatened release of hazardous materials, including, but not limited to, the following: <br /> - immediate notification of local emergency personnel and the Environmental Health Department (EHD) <br /> - procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br />persons, property, or the environment <br /> - evacuation plans and procedures, including immediate notice, for the site <br />Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the <br />correct or updated information, and submit to the EHD for approval. <br />This is a Class II violation. <br /> 9 HSC 25508(a)(1) Failed to annually review and certify that the business plan is complete, accurate, and up to date. <br />The business plan information has not been reviewed and resubmitted in the California Environmental Reporting <br />System (CERS) annually. The hazardous materials inventory shall be submitted by January 15 of each calendar <br />year and may be submitted beginning November 1 of the previous year. Immediately log into the CERS at <br />http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br />This is a Class II violation. <br /> 11 HSC 25505(a)(4) Failed to provide or document initial and/or refresher training to appropriate personnel. <br />No current training records available for inspection. The business plan shall include provisions for ensuring that <br />appropriate personnel receive initial and annual refresher training. This training shall be documented electronically <br />or by hard copy and shall be made available for a minimum of three years.
