STOCKTON September 07, 2018 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS 11,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 11 HSC 25505(a)(4) Failed to provide or document initial and/or refresher training to appropriate personnel. <br /> Employee training records were not available at the time of inspection. The business plan shall include provisions <br /> for ensuring that appropriate personnel receive initial and annual refresher training This training shall be <br /> documented electronically or by hard copy and shall be made available for a minimum of three years Immediately <br /> provide employee training for appropriate personnel and submit a copy of the training records to the EHD. <br /> This is a Class II violation. <br /> 12 HSC 25505(c) Failed to have the business plan readily available to facility or EHD personnel, <br /> Facility could not provide the Emergency Response Plan and the Chemical inventory upon request. The emergency <br /> response plans and procedures, the inventory of information required by this article, and the site map required by <br /> this section shall be readily available to personnel of the business or the unified program facility with responsibilities <br /> for emergency response or training pursuant to this section. Immediately make arrangements to have the business <br /> plan readily available for review. <br /> This is a minor violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, within 30 days of receiving the completed inspection report. <br /> Please be aware as of January 1, 2013, all businesses were required to submit all hazardous materials <br /> information online to the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov- Be <br /> sure to include your hazardous material activity in the Businesses Activities section in CERS in addition to any <br /> other relevant activities and required fields. <br /> For assistance with CERS, contact Toby Morelli at tmorelli@sjcehd.com or 209-468-3421. <br /> AFTER THE COMPLIANCE DATE, EHD WILL BILL FOR ALL TIME AND ACTIVITIES ASSOCIATED WITH BRINGING <br /> THIS FACILITY BACK INTO COMPLIANCE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> Received by: Date: September 07, 2018 Inspector.
