The following is an itemized list of hazardous materials violations that have not been <br /> addressed for WHISKEY SLOUGH HARBOR as of December 112018. <br /> Open violations from May 29, 2018 inspection <br /> Violation#5 - Failed to complete and/or submit hazardous materials inventory for all reportable materials on <br /> site. <br /> A 278 c.f. cylinder of compressed oxygen and a tank of diesel (greater than 55 gallons)were found on site that has <br /> not been reported. Any material that meets or exceeds the reportable quantity shall be reported in the facility's <br /> business plan. Immediately log into the California Environmental Reporting System (CERS) at <br /> http://cers.calepa.ca.gov/, add the materials to the hazardous material inventory, and submit to the EHD for <br /> approval. <br /> Violation #6 - Failed to complete and/or submit a site map with all required content. <br /> The site map did not show north orientation, loading areas, adjacent streets, storm and sewer drains, emergency <br /> shutoffs, evacuation staging areas, hazardous material handling and storage areas, and emergency response <br /> equipment. A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and <br /> sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling <br /> and storage areas, and emergency response equipment. Immediately log into the California Environmental <br /> Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the <br /> EHD for approval. <br /> Violation#7 - Failed to establish and/or submit emergency response procedures for a release or threatened <br /> release. <br /> The emergency response procedures have not been submitted. A business plan shall include the following <br /> emergency response procedures for a release or threatened release of hazardous materials, including, but not <br /> limited to, the following: <br /> - Immediate notification of local emergency personnel and the Environmental Health Department(EHD) <br /> - Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment <br /> - Evacuation plans and procedures, including immediate notice, for the site <br /> Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the <br /> correct or updated information, and submit to the EHD for approval. <br /> Violation#8 -Failed to submit a training program or program is not reasonable or adequate for facility. <br /> A training program has not been submitted. Training programs for all employees and annual training, including <br /> refresher courses, shall include safety procedures for the event of a release or threatened release of a hazardous <br /> material, including, but not limited to, familiarity with the facility's emergency response plans and procedures. These <br /> training programs may take into consideration of each employee. Immediately prepare or update the facility's <br /> training program, log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, <br /> upload the correct or updated information, and submit to the EHD for approval. <br /> Violation#9 - Failed to annually review and certify that the business plan is complete, accurate, and up to <br /> date. <br /> The business plan information has not been reviewed and resubmitted in the California Environmental Reporting <br /> System (CERS) annually. The hazardous materials inventory shall be submitted by January 15 of each calendar <br /> year and may be submitted beginning November 1 of the previous year.
