The site map is missing emergency response <br />equipment locations ( fire extinguisher), storm drain location, and an evacuation staging area, utilities shut offs, <br />loading and unloading area for the chemicals. <br /> A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer <br />drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and <br />storage areas, and emergency response equipment. If a site map element is not applicable for your facility then list <br />it on the map and label as “NA". Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br />This is a repeat violation, Class II. <br /> 7 HSC 25505(a)(3) Failed to establish and/or submit emergency response procedures for a release or threatened <br />release. <br />The emergency response procedures are not complete. The local Unified Program Agency (UPA) phone number <br />was not provided in section 3C and please fill out section H. Please add 209-468-3420 as the local UPA phone <br />number. <br /> A business plan shall include the following emergency response procedures for a release or threatened release of <br />hazardous materials, including, but not limited to, the following: <br /> - immediate notification of local emergency personnel and the Environmental Health Department (EHD) <br /> - procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br />persons, property, or the environment <br /> - evacuation plans and procedures, including immediate notice, for the site <br />Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the <br />correct or updated information, and submit to the EHD for approval. <br />This is a Class II violation. <br /> 9 HSC 25508(a)(1) Failed to annually review and certify that the business plan is complete, accurate, and up to date. <br />The business plan information has not been reviewed and resubmitted in the California Environmental Reporting <br />System (CERS) annually. The last submittal was made on 5/11/2017. Please make a submittal for 2018. The <br />hazardous materials inventory shall be submitted by January 15 of each calendar year and may be submitted <br />beginning November 1 of the previous year.
