Emergency Action Plan Page 4 of 7 <br />o Only under specific circumstances where the fire can be characterized as in the "incipient stage" and <br />trained personnel with fire extinguisher experience, should ERT members attempt to contain the fire. At <br />this same time, all other personnel are expected to follow evacuation procedures. <br />o Note: Do not attempt to fight any fire without first sounding an alert. Trained personnel must first <br />determine the type of fire and the proper type of extinguisher to use before attempting to control with <br />a fire extinguisher. If unable to make this determination, all remaining personnel must evacuate the <br />facility immediately, without further attempt to extinguish. <br /> <br /> Earthquake - The following are procedures designed to minimize employee risk of injury during an earthquake. <br />Contingency plans are being developed to address preparedness issues should an earthquake limit personnel <br />contact with outside emergency agencies for an extended period of time. The basic reactive procedures during <br />an Earthquake are: <br />o Drop and take cover under a strong/durable surface and grasp hold of a fixed object to minimize further <br />movement [concern is from falling objects from overhead] <br />o Take cover under a doorway and hold on to the door frame for balance <br />o If unable to take cover, crouch down low to the ground, tuck head between knees, and cover top of <br />head with hands <br />o Avoid cover near/under overhead racks, electrical panels, gas piping, or other potentially hazardous <br />objects (concern is from fire/explosive hazards) <br />o Do not attempt to run through the building <br />o Once earthquake has subsided, notify others of your whereabouts (if injured or trapped) and wait help if <br />injured <br />o ERT will advise if/when necessary and safe to evacuate the building <br />o Proceed with evacuation procedure, but only when safe to do so <br />o Leave the building, only if safe to do so (motion has subsided) and clear the outside structure and all <br />other structures by at least 100 feet <br />o Advise ERT of personnel (and their locations) who may be injured or trapped <br />o ERT Coordinator instructs other team members needed to assist using cellular phone communication <br />devices only. Coordinator to designate member to follow Operation Shutdown procedures, as needed. <br />Coordinator to be advised upon completion of Shutdown procedures. Coordinator to also assign two- <br />person teams to tour facility and identify locations where personnel may be injured or trapped. <br />Coordinator to advise when safe and appropriate to evacuate building, then give verbal command (via <br />networking through ERT and department contacts). <br />o Follow up inspection of complete facility will be required (performed by Maintenance and/or Facilities <br />Manager), paying close attention to key areas defined on pre-defined inspection form. <br />o In addition, the inspection will look for and attempt to identify if the structural integrity of the building, <br />equipment, and storage racks have been compromised in any way to jeopardize employee safety. <br />o A full written report will follow with findings and recommendations submitted to the Safety Manager <br />and appropriate management. In areas found to be hazardous, will be isolated and cordoned off from <br />employee access until full and complete repairs are completed. <br /> <br /> Hazardous Materials/Chemical Spill <br />o It is our company policy that not more than one gallon of any hazardous material will be stored on the <br />production floor (outside of secured containment areas) at any given time, hazardous material spills <br />constituting an emergency response will be reported immediately to outside agencies. <br />o All emergency response hazardous material spills will be reported to the City of San Jose Fire <br />Department. Internal response procedures for handling "incidental releases" of hazardous spills which <br />employees have been previously trained are, as follows: <br />o ERT Coordinator will order an evacuation of the hazardous area surrounding the spill, eliminate the <br />possibility of fire/explosion by de-activating power sources (i.e. electrical power and/or gas supply to the <br />area), and remove non-compatible materials or objects that could potentially increase the hazard.
