The site map did not include a legend/key to identify the codes used to denote hazardous material storage containers. A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, and emergency response equipment. Immediately log into the California Environmental Reporting System(CERS)at http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. This is a Class II violation. 7 HSC 25505(a)(3) Failed to establish and/or submit emergency response procedures for a release or threatened release. The emergency response procedures were lacking or inadequate. The phone number for the CUPA is not 209-953-6200,the name and phone number of the nearest medical facility/hospital has not been identified,the phone number of the RWQCB has not been identified and the vulnerable systems in Section H have not been completed. A business plan shall include the following emergency response procedures for a release or threatened release of hazardous materials, including, but not limited to, the following: -immediate notification of local emergency personnel and the Environmental Health Department (EHD) -procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to persons, property, or the environment -evacuation plans and procedures, including immediate notice, for the site Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. This is a Class II violation. 9 HSC 25508.2 Failed to annually review and certify that the business plan is complete, accurate, and up to date. The business plan information has not been reviewed and resubmitted in the California Environmental Reporting System(CERS)annually. The hazardous materials inventory shall be submitted by January 15 of each calendar year and may be submitted beginning November 1 of the previous year. Immediately log into the CERS at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. This is a Class II violation. Overall Inspection Comments: Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement documenting the corrective actions that have been or will be taken for each violation, and any supporting paperwork, within 30 days of receiving the completed inspection report. Please be aware as of January 1, 2013, all businesses were required to submit all hazardous materials informatio online to the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov.
