Failed to provide appropriate containment to prevent a discharge from a primary containment <br /> system. A secondary containment testing was performed on 05/2212009. Test results indicated that <br /> secondary and sumps 3 & 4 failed. There is a breach in the diesel pipe run system and it is not tight. The <br /> entire containment system must be constructed so that any discharge from primary containment system <br /> will not escape the containment system before cleanup occurs. 'General' containment per CFR 112.7(c) <br /> may be active or passive in design, use or operation, and must be sufficient to contain a release from <br /> the 'typical' failure mode and likely release volume. Correct by having a license, trained and certified <br /> technician repair the failed component returning the containment system into proper maintenance. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($115). <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector: Received By: Title: <br /> CONTINUATION FORM <br /> EHD 23-02-003 Rev 0610 110 <br />
