Section 2.4 of revised Plan. <br /> Violations # 716 & 717 referring to high level monitoring and liquid level sensing devices are in <br /> process of being corrected by purchasing replacement or new sensing devices. All tanks monitoring <br /> systems have been checked and inoperable devises will be purchased and installed within 60 days. <br /> Violation #727 referring to buried piping intregity testing —The records of intregity testing at time of <br /> install are not available at this time. DVI is actively searching for these documents however the <br /> underground piping inspections will be performed as required. Where underground piping exists, a <br /> contract to have the lines tested will be developed and procured. The contract process may take as <br /> long as 120 days. Alternate proposals will be to replace underground piping with above ground <br /> piping where possible. <br /> #728—CFR 112.8(d)(5)—failed to adequately warn vehicle entering facility to protest piping <br /> and other transfer operations. <br /> In respond to this violation be advices that all existing tanks have proper warning signage for tank <br /> content and hazard and also NFPA 704 sign, including tanks in question: <br /> Fresh water booster station — 400 gal tank has proper signage. Tank is located approximately <br /> 50 feet from the road with vehicles access limited by existing water valves and bollards. Photo is <br /> attached. <br /> PIA Farm garage- 2000 gal tank has proper signage and traffic bollards on all sides of tank. <br /> Photo is attached. <br /> PIA Dairy Production—400 gal belly tank is located on the grassy lawn with no vehicle access. <br /> Vehicle protection is provided by generator package and has proper signage. <br /> However in order to meet requirements installation of traffic bollards will be discussed with <br /> PE at the time of certification of revised Plan. <br /> Waste Water Treatment Plant— 150 gal belly tank is located approximately 10 feet from the curb <br /> and has proper signage. Vehicle protection is provided by generator package. <br /> However in order to meet requirements installation of traffic bollards will be discussed with <br /> PE at the time of certification of revised Plan. <br /> In Summary, DVI has taken corrective action on the violations listed above, however there are <br /> outstanding issues that are dependent on the procurement and ordering process. DVI recognizes <br /> that there is a need to be fully compliant with regulations and requests the opportunity to perform <br /> the necessary corrections within the timeframes identified. Your assistance and patience will be <br /> greatly appreciated. If the proposed corrections are not acceptable, DVI will diligently work to meet <br /> the deadlines provided by your office.
