Failed to conduct periodic inspection of storage tanks. SPCC plan states facility must conduct <br /> written daily inspections. Upon reviewing the records, the daily inspections are not documented daily <br /> and tanks are not inspected daily. Each owner/ operator shall conduct periodic inspections of storage <br /> tanks to assure compliance with CFR Part 112. Correct by conducting regular inspections and maintain <br /> documents with the Plan on site for a minimum of 3 years. <br /> 510. SPCC Plan failed to include required contact list and phone numbers of facility's response <br /> coordinator, clean up contractors, and agencies to notify in case of discharge. SPCC plan contact list <br /> currently lists only the state agencies and facility's management for discharge spillage. It lacks contact <br /> phone number of local agencies such as SJC EHD. The SPCC Plan must include a contact list and <br /> phone numbers for the facility response coordinator, National Response Center, cleanup contractors <br /> with whom you have an agreement for response, and all appropriate federal, state, and local agencies <br /> who must be contacted in case of a discharge. Immediately update the contact list to include the most <br /> current version of the required information. <br /> 518. Failed to train personnel on discharge prevention on an annual basis. Last annual refresher was <br /> conducted 01/2012 and is currently five months past due. At a minimum, oil-handling personnel must be <br /> trained in the operation and maintenance of equipment to prevent discharges, discharge procedure <br /> protocols, applicable pollution control laws, rules & regulations, general facility operations, and the <br /> contents of the facility's SPCC Plan. Annual briefings must be conducted to assure adequate <br /> understanding of the SPCC Plan and highlight and describe any known discharges or failures, <br /> malfunctioning components, and any recently developed precautionary measures. Immediately conduct <br /> necessary training for all oil handling personnel. Submit copies of training records to the EHD. <br /> 0esig <br /> Failed to designate a person to be accountable for discharge prevention. SPCC plan lacks an <br /> nated individual listed to be accountable for discharge prevention. Designated person is <br /> accountable for discharge prevention and is responsible for reporting to facility management.
