The business plan shall include provisions <br />for ensuring that appropriate personnel receive initial and annual refresher training. This training shall be <br />documented electronically or by hard copy and shall be made available for a minimum of three years. Immediately <br />provide employee training for appropriate personnel and submit a copy of the training records to the EHD. <br />This is a Class II violation. <br />12 <br />HSC 25505(c) Failed to have the business plan readily available to facility or EHD personnel. <br />Facility could not provide the Emergency Response Plan and the Chemical inventory upon request. The emergency <br />response plans and procedures, the inventory of information required by this article, and the site map required by <br />this section shall be readily available to personnel of the business or the unified program facility with responsibilities <br />for emergency response or training pursuant to this section. Immediately make arrangements to have the business <br />plan readily available for review. <br />Corrected on site. <br />This is a minor violation. <br />Overall Inspection Comments: <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting <br />paperwork, within 30 days of receiving the completed inspection report. <br />Assisted with password reset and CERS access at the time of inspection. <br />Email address: jamesfortune60@gmail.com <br />Facility does not have loading areas and emergency shutoffs and these elements were not addressed on the <br />site map. Please address this by either writing "NA" for not applicable or a statement that says the element is <br />not on site.
