Ensure that no materials which are incompatible with any released materials are <br />treated, stored, or disposed of, until cleanup procedures are completed. <br />c. Ensure that all emergency equipment is cleaned or replaced and is fit for use before <br />operations are resumed. <br />d. Prior to resuming operations, notify the Fire Department and the Department of Toxic <br />Substances Control that the facility is ready to resume operations. <br />e. Prepare an Emergency Event Investigation Form (500-0010-002) and submit a report <br />to the Fire Department, Cal-EMA and to the Department of Toxic Substances Control <br />within 15 days of the incident, which includes the following information: <br />Name, address, telephone number of the facility owner/operator. <br />Name, address, telephone number of the facility. <br />Date, time and type of incident. <br />Name and quantity of material(s) involved. <br />The extent of injuries, if any. <br />An assessment of actual or potential hazards to human health or the <br />environment. <br />An estimate of the quantity and disposition of recovered material that <br />resulted from the incident (if any). <br />REGENEv <br />MAy i a 205 <br />EN�IRONMENTA� <br />
