Weber Avenue <br />Stockton, Ca. 95201 <br />FROM: Letitia Resch, REHS P <br />INSPECTED BY: Letitia Resch, REHS, Public Health Services - <br />Environmental Health Division. <br />DATE OF INSPECTION: November 5, 1990 <br />DATE OF REPORT: <br />I. PURPOSE: <br />January 15, 1991 <br />An inspection was conducted at Holly Sugar, due to an <br />anonymous complaint received by Public Health Services - <br />Environmental Health Division (PHS-EHD), on November 5, 1990 <br />at 1:30 a.m. The complaint alleged Holly Sugar had an <br />unreported spill of approximately 1,000 gallons from their <br />above ground fuel tank. <br />II. INVESTIGATION: <br />On November 5, 1990 at 11:30 a.m. I arrived at the Holly Sugar <br />plant gate and identified myself to the guard. The guard <br />arranged for the factory manager to meet me. The manager <br />identified himself by card as David M. Duncan. Next, I <br />followed Mr. Duncan's white pickup to a location northeast of <br />the gate where the above ground fuel tank was located. Mr. <br />Duncan and I exited our vehicles and walked over to the dirt <br />bermed area around the tank. The bank of the dirt berm was <br />approximately 6 to 8 feet above the spill. The spill was <br />clearly visible, it was puddled around the tank and seeping <br />out from where it had been covered with soil. There was a <br />large black stain on the west side of the tank. Mr. Duncan <br />identified this stain as the place where workmen broke off the <br />tank spout during a weed abatement operation. The spill <br />happened on November 4, 1990 at approximately 3:00 p.m. in the <br />A Division of San Joaquin County Health Care Services 0 <br />
