Diesel fuel was observed today in the ;ander dispenser containments 3 and 11 Any release: from <br />the primary containment that does not escape the secondary containment and was cleaned up within <br />eight hours of being detected shall be recorded wl l the following information. <br />1.Operator's name and phone number <br />2: List of types, € uantities, and concentrations of hazardous substances released <br />3. Description of actions taken to control and clean the release <br />4. Method and location of disposal of the released substances <br />. Description of actions taken to repair and prevent future release$ <br />6, Description of method used to reactivate interstitial monitoring after repair or replacement of <br />primary containment <br />Immediately record this release with the information required, and maintain it on site with the facility's <br />monitoring records. Submit a copy of the recordable release statement to the EI' D by May 24, 2013. <br />303. The (diesel rill sump liquid sensor was located approximately six inches off the lowest point of the <br />sump and not located to detect a leak at the earliest opportunity. Monitoring equipment shall be <br />maintained to be able to detect a leak at the earliest possible opprar umly, The technician placed the <br />liquid sensor at the lowest point of the sump. Violation resolved <br />311, The Diesel line leak detector failed to detect a leak when tested. All line leak detectors shell be <br />capable of detecting a 3 -gallon per hour leer at 10 psi, Immediately have a properly licensed, trained. <br />and certified contractor repair or replace the failed leak. detector (LG 113-x listed, if applicable) and <br />obtain a permit within one business day from the EFT. If the failed lean detector can't be repaired <br />immediately, there is a possibility that the Diesel UST system may be red tagged to prevent fuel inputs, <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BitID AT THE CURRENT HOURLY RATE. i <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />
