Contact the local fire department and the San Joaquin Valley Air Pollution Control District <br />for any applicable permits and requirements. <br />2. Any changes in information and scope of work must be submitted to EHD for approval <br />prior to commencing work. All new addenda supersede the previous ones. <br />3. The excavation must be secured if it will not be backfilled'imnfediately. <br />4. If the backfill material and soil is found to be contaminated it must be disposed of at an <br />approved disposal location. Treatment of soil onsite can only be -performed under <br />hazardous waste treatment permit. <br />5. See pages 7 and 8 of the permit application for required number and locations of <br />analytical tests. <br />6. EHD must witness the soil sampling. Schedule the soil sampling appointment with this <br />office 48 hours in advance. <br />7. Schedule removal appointment 48 hours in advance. <br />8. All personnel involved in the removal of the existing pipes must fulfill requirements of <br />CCR title 8 Section 5192. <br />9. A copy of the submitted "Health and Safety Plan" must be, on the jobsite for review. i <br />In addition, the following information must be submitted within 30 days of.the removal date. <br />(a) Analytical results, mailed from the lab <br />(b) UST Tracking Record Sheet and Tank Hazardous Waste Manifest for each UST <br />(c) Tank Closure Certification Form for each UST <br />(d) Hazardous Waste Manifest for piping, rinsate, residual fuel, and if necessary <br />backfill material. <br />(e) Submit the backfill excavation certificate as required by San Joaquin County. <br />If you have any questions, please contact me at (209) 468-3442. <br />Thank you, <br />Harlin Knoll, R.E.H.S. <br />Senior Registered Environmental Health Specialist <br />Cc: Anne Ogrey-Holly Sugar <br />20500 Holly Drive, Tracy <br />
