Wilson, R.E.H.S. <br />Margaret Lagorio, R.E.H.S. <br />Robert McClellon, R.E.H.S. <br />Mark Barcellos, R.E.H.S. <br />A recent review of the underground storage tank file for the above referenced facility indicated this office has <br />not received this years testing and paperwork requirements. <br />The following information is overdue: <br />_ HSC 25292.2 Certification of financial responsibility and letter from chief financial officer <br />_ CCR 2637(b) Annual monitoring certification <br />_ CCR 2336(f)(4) Annual line tightness test (If applicable) <br />_ HSC 25284.2 Annual spill bucket/containment test <br />_ CCR 2630(d) Annual line leak detector test <br />Note: <br />_ CCR 2637(a) 36 -month secondary containment test is due on or before 12/31/05 <br />Most forms may be accessed from our web site address, aWW%teocides.comfunitiii <br />In addition, this office must be notified of any test date_ and time within 48 -hours of performing the test. <br />Furthermore, results from the testing must be sent to this office within 30 days of the test completion. Under <br />CCR 2637(b)(4) it is the owner/operators responsibility to submit test results to this office.
