Current financial responsibility documents on file with the EHD - Repeat violation. Upon reviewing the file for this facility, current financial responsibility documents were found not to be on file. Updated financial responsibility documents must be submitted to the EHD annually. The current financial responsibility document was found on site and a new cover page document was filled out. Corrected on site. #5. Monitoring Program (Monitoring & Response Plans) is not current or approved by the EHD - Repeat violation. Upon reviewing the file for this facility current Monitoring & Response Plans were not found. The Monitoring & Response Plans must be resubmitted on the current forms. I provided a blank copy which was filled out by the Facilities Manager. Retain a copy on site for use by facility employees. Corrected on site. Note: Secondary Containment testing for this facility was last tested in September 2007. The test must be scheduled by the end of this month with the EHD giving 48 hour notification or otherwise this facility will be out of compliance. Employee training by the designated operator is due this month. ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE ($122).
