The first did not include the Satellite 3 piping run which was inadvertently left off. Apparently I saved and sent the wrong one. Attached is the corrected one they sent out. The violations that are still open are: 107 , 203. For violation 107, 1 have attached a highlighted site map of how to correct the violation. The VMI LD2000 leak detector need to be added onto the site plan. Once you have made the correction, upload the new site plan to CERS. For violation 203, you need to keep records of the 2014 and 2015 annual monitoring certification results onsite, and send a statement saying you will keep the monitoring certification test results onsite. Also, after reviewing the 6 month retest of the Secondary Containment Test results performed on 11/15/16, it looks like Satellite piping 1, and satellite piping 2 were tested. Satellite piping 3 needs to be tested. Schedule a secondary containment test for satellite piping 3 and notify the EHD of the testing. If you have any questions, please contact me.
