Piping replacement was done on the satellite #1 piping run on June 7, 2012 and tightness testing <br />results have not been submitted prior to operating the tank. Immediately provide copies of the test <br />results to the EHD. REPEAT VIOLATION <br />204. The designated operator failed to document all the alarms from the attached alarm history on the <br />April 2013 designated operator monthly inspection report and failed to check that they were responded <br />to appropriately. The missing alarms include: C4 (UDC 3/4) on March 27, 2013 and April 4, 2013. <br />During the monthly inspection, the designated operator shall review the alarm history for the previous <br />month, check that each alarm was documented and responded to appropriately, and attach a copy of <br />the alarm history with documentation taken in response to any alarms to the monthly report. Ensure that <br />designated operators performing monthly inspections at this facility are including all of the required <br />information on the reports. <br />Complete and submit a copy of the Return to Compliance Certification to the EHD by April 14, 2014, <br />along with a statement documenting the corrective actions that have been or will be taken for each <br />violation. <br />Note: During replacement of the under dispenser containments (UDCs), the UDC 3/4 sensor stopped <br />working properly. According to the technician and the printed alarm history, this sensor went into alarm <br />at 7:45 AM this morning and cleared, but would not go into alarm during pre -testing. The sensor was <br />replaced prior to the completion of the construction project and the routine inspection. A "UST Retrofit <br />Verification with Inspector Already Onsite" form has been completed and provided to the operator and <br />contractor.
