Piping replacement was done on the satellite #1 piping run on June 7, 2012 and tightness testing results have not been submitted prior to operating the tank. Immediately provide copies of the test results to the EHD. REPEAT VIOLATION 204. The designated operator failed to document all the alarms from the attached alarm history on the April 2013 designated operator monthly inspection report and failed to check that they were responded to appropriately. The missing alarms include: C4 (UDC 3/4) on March 27, 2013 and April 4, 2013. During the monthly inspection, the designated operator shall review the alarm history for the previous month, check that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. Complete and submit a copy of the Return to Compliance Certification to the EHD by April 14, 2014, along with a statement documenting the corrective actions that have been or will be taken for each violation. Note: During replacement of the under dispenser containments (UDCs), the UDC 3/4 sensor stopped working properly. According to the technician and the printed alarm history, this sensor went into alarm at 7:45 AM this morning and cleared, but would not go into alarm during pre -testing. The sensor was replaced prior to the completion of the construction project and the routine inspection. A "UST Retrofit Verification with Inspector Already Onsite" form has been completed and provided to the operator and contractor. Log into the California Environmental Reporting System (CERS) and update the monitoring plan for this site to represent the new sensor.
