Tracy Program: APSA <br /> SUMMARY OF VIOLATIONS <br /> CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Routine Aboveground Petroleum Storage Tank inspection report <br /> 517. Copies of the quarterly inspection for the aboveground containers were not found on site. <br /> Inspections and tests must be conducted in accordance with the written procedures developed in the <br /> SPCC Plan. Records of these inspections and tests must be signed by the appropriate supervisor or <br /> inspector and kept on site with the SPCC Plan for a period of three years. Immediately begin conducting <br /> the quarterly inspection, locate any copy of all inspection and testing records for the last three years, and <br /> maintain them on site. <br /> 518 (2). This facility does not have a designated person for discharge prevention. The SPCC Plan shall <br /> designate a person at the facility who is accountable for discharge prevention and who reports to facility <br /> management. Immediately designate a person to be accountable for discharge prevention and update <br /> the SPCC Plan to include this information. <br /> 513. The Plan lacks description of equipment failure for above ground containers/ equipments. <br /> Discharge & drainage controls and potential equipment failure are not addressed for any of the unlisted <br /> containers. The SPCC Plan must include the correct type of oil and storage capacity of each fixed <br /> storage container, and either the type of oil and storage capacity of each portable container or an <br /> estimate of the potential number of portable containers, types of oil, and anticipated storage capacities. <br /> The Plan must include discharge or drainage controls, such as secondary containment around all <br /> containers and other structures, equipment, and procedures for the control of a discharge, and a <br /> prediction of the direction, rate of flow, and total quantity of oil which could be discharged as a result of <br /> each type of major equipment failure. Immediately update the discharge information in the SPCC Plan <br /> to accurately represent the petroleum storage at the facility. <br /> 519. 49. The SPCC Plan did not include discussion of adequate security lighting. The Plan must include <br /> descriptions of how you secure and control access to the oil handling, processing & storage areas, <br /> secure master flow & drain valves, prevent unauthorized access to starter controls on oil pumps, secure <br /> out-of-service and loading/unloading connections of oil pipelines, and address the appropriateness of <br /> security lighting to both prevent acts of vandalism and assist in the discovery of oil discharges. <br /> Immediately update the Plan to include all of the required security information. <br /> Submit to SJC EHD within 30 days (10/7/11): <br /> 1. one copy of the completed "Return to Compliance Certification" form <br /> 2. a written corrective action statement for each of the above violation <br /> Do not submit a copy of the SPCC Plan to EHD.
