El Dorado St., Stockton,CA <br /> (RWQCB Case#391005) <br /> Dear Ms. Duncan: <br /> Enclosed herewith is a completed drilling permit application and associated fee for the destruction <br /> of seven groundwater monitoring wells at the subject site(Figure 1). The work will be conducted in <br /> accordance with Ground Zero's December 17, 2008, Workplan for Monitoring Well Destruction, <br /> which was approved by the California Regional Water Quality Control Board (RWQCB) in <br /> correspondence dated February 3, 2008 and February 23, 2008, both of which are included in <br /> Attachment A. <br /> Well destruction activities will be conducted by Fisch Drilling (C57-683865) under the <br /> supervision of a Ground Zero geologist. Groundwater monitoring wells MWl through MW7 <br /> will be destroyed by overdrilling with 8-inch diameter hollow stem augers and the resulting annulus <br /> will be backfilled with neat cement grout emplaced through a tremie pipe system. The surface will <br /> be restored to match the surrounding material. The monitoring wells are all 2-inches in diameter <br /> and vary in depth from 32 feet below the ground surface (bgs) to 38 feet bgs. Monitoring well <br /> construction details are summarized in Table 1 and are provided in more detail in the well logs <br /> included in Attachment B. Prior to any well destruction activities the wells will be sounded to <br /> insure no obstructions are present in the wells. The locations of the monitoring wells are shown on <br /> the site plan provided as Figure 2. <br /> Drill cuttings and debris will be drummed and a composite sample will be collected for waste <br /> profiling purposes. Following the receipt of analytical results the waste will be profiled and <br /> properly disposed of. <br /> All field work will be conducted in accordance with the procedures described in the Quality <br /> Assurance/Quality Control Plan and Site Safety Plan on file with RWQCB and the San Joaquin <br /> County Environmental Health Department(EHD). <br /> G:\GROUNDZE\GABBARD\Closure\Permits\wellDmtPeimit.doc <br />
