CA 95240 Program: APSA <br /> SUMMARY OF VIOLATIONS <br /> CLASS I,CLASS II, or MINOR-Notice to Comply) <br /> #27 The facility SPCC Plan failed to include a description of potential equipment failure(direction, flow <br /> rate, quantity). Where experience indicates a reasonable potential for equipment failure (such as loading <br /> or unloading equipment, tank overflow, rupture, or leakage, or any other equipment known to be a <br /> source of a discharge, include in your Plan a prediction of the direction, rate of flow, and total quantity of <br /> oil which could be discharged from the facility as a result of each type of major equipment failure. <br /> Correct immediately by incorporating the required description of potential equipment failure into the <br /> facility SPCC Plan. <br /> #31 The documentation for the Ten Year Suitability Testing was not available for review. Per the facility <br /> SPCC Plan on page eleven, "The above ground tank will undergo pressure or ultrasonic integrity testing <br /> every ten years". This plan has a reviewed/revised date of September 1999. Correct immediately by <br /> locating the test report for the ten year pressure or ultrasonic integrity testing and maintain it available for <br /> review for at least three years. <br /> #33 The security measures to address the diesel dispenser, security lighting, security of loading and <br /> unloading connections are not in the plan. The SPCC Plan must describe how you secure and control <br /> access to the oil handling, processing and storage areas; secure master flow and drain valves; prevent <br /> unauthorized access to starter controls on oil pumps: secure out-of-service and loading/unloading <br /> connections of oil pipelines; and address the appropriateness of security lighting to both prevent acts of <br /> vandalism and assist in the discovery of oil discharges. Correct immediately by incorporating the <br /> required description of security measures into the facility SPCC Plan. <br /> Complete and Submit the return to compliance certification, corrective action statement for each <br /> violation and any supporting documentation to this office by 1-13-11. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122). <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspe Receive dlJ � Title 5 <br /> EHD 23-02-003 Rev 08/10/10 CONTINUATION FORM <br />
