Where experience indicates a reasonable potential for equipment failure (such as loading or unloading equipment, tank overflow, rupture, or leakage, or any other equipment known to be a source of a discharge, include in your Plan a prediction of the direction, rate of flow, and total quantity of oil which could be discharged from the facility as a result of each type of major equipment failure. Correct immediately by incorporating the required description of potential equipment failure into the facility SPCC Plan. #31 The documentation for the Ten Year Suitability Testing was not available for review. Per the facility SPCC Plan on page eleven, "The above ground tank will undergo pressure or ultrasonic integrity testing every ten years". This plan has a reviewed/revised date of September 1999. Correct immediately by locating the test report for the ten year pressure or ultrasonic integrity testing and maintain it available for review for at least three years. #33 The security measures to address the diesel dispenser, security lighting, security of loading and unloading connections are not in the plan. The SPCC Plan must describe how you secure and control access to the oil handling, processing and storage areas; secure master flow and drain valves; prevent unauthorized access to starter controls on oil pumps: secure out-of-service and loading/unloading connections of oil pipelines; and address the appropriateness of security lighting to both prevent acts of vandalism and assist in the discovery of oil discharges. Correct immediately by incorporating the required description of security measures into the facility SPCC Plan. Complete and Submit the return to compliance certification, corrective action statement for each violation and any supporting documentation to this office by 1-13-11. ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122).
