The Report was submitted in response to the <br /> 1 August 2008 Regional Board Executive Officer's order (Order) issued pursuant to California <br /> Water Code section 13267. The Report provides the complete chain of title and legal <br /> description for the property, and fulfills the Order Monthly Status Report requirement for <br /> November 2008. Regional Board staff find that, with the submittal of the Report and prior <br /> reports, the intent of the Order to move this case towards closure has been met, and now <br /> recommend suspension of said remaining Order requirements for quarterly monitoring and a <br /> monthly status report pending closure review. Therefore, in order to proceed into the public <br /> participation phase for the closure: <br /> 1. Effective immediately, the Fourth Quarter 2008 quarterly groundwater monitoring and the <br /> December 2008 Status Report are suspended. <br /> 2. You are to submit a workplan for monitoring well abandonment. <br /> 3. You are to submit a property parcel map depicting your site, the extent of your identified <br /> petroleum hydrocarbons plume, and all parcels located within 200 feet of your site and/or <br /> your defined hydrocarbon plume. For each parcel, located within 200 feet of your site <br /> and your defined hydrocarbon plume, you are to provide the parcel number, the name of <br /> the owner for each parcel, and a mailing address for each owner. <br /> 4. You are to submit a public notification document (Fact Sheet) for our review. The Fact <br /> Sheet should contain a very brief site history, discussing investigative and remedial <br /> activities conducted at the site, current site conditions, and your findings that the residual <br /> constituents mass remaining in soil and groundwater beneath your site does not pose a <br /> threat the human health or waters of the state. The Fact sheet must also state your <br /> intent to obtain site closure from the Regional Board, and include a method by which any <br /> interested individuals may obtain additional information regarding the site or make <br /> comments regarding the possible closure of the site. I have informed your consultant <br /> Ground Zero Analysis Inc. of our requirements.
