Notify Prime Shine maintenance department and Prime Shine headquarters. <br /> Step 6. Notify Office of Emergency Services (see list of agencies in "Emergency <br /> Services" section). <br /> II. Post Emergency <br /> Step 1. Make arrangement for treating, recovering and transporting any waste involved <br /> in the emergency. <br /> Step 2. Ensure that all equipment and machinery affected by emergency has been <br /> returned to perfect operating condition. <br /> Step 3. Ensure that all emergency equipment listed in the contingency plan is clean and <br /> fit for its intended use before facility operations resume. <br /> Step 4. Contact Environmental Health Department and appropriate state and local <br /> agencies that no incompatible wastes are in contact with the affected areas and <br /> all emergency equipment listed in the contingency plan is ready for use. <br /> Step 5. Thoroughly document emergency using Emergency Information Form located in <br /> contingency plan notebook. <br /> Step 6. Within 15 days after the incident submit a report to the Environmental Health <br /> Department. Copies found in front of Haz Mat binder stored at the desk. Use the <br /> Emergency Reporting Form located in the contingency plan notebook. The report <br /> shall include: <br /> (1) name, address, and telephone number of the owner or operator; <br /> (2) name, address, and telephone number of the facility; <br /> (3) date, time, and type of incident(e.g., fire, explosion); <br /> (4) name and quantity of material(s) involved; <br /> (5) the extent of injuries, if any; <br /> (6) an assessment of actual or potential hazards to human health or the <br /> environment, where this is applicable; and <br /> (7) estimated quantity and disposition of recovered material that resulted from the <br /> incident <br /> 3. Non-Emergency Procedures- If emergency does not threaten human life or the environment <br /> proceed with the following steps. (ie: minor floor spill) <br /> Step 1. Determine nature of event and why incident has occurred. <br /> Step 2. Use appropriate procedures and equipment provided to clean up incident. <br /> Coordinator will determine if professional services are required to properly clean <br /> up. <br /> Step 3. If equipment has malfunctioned contact equipment repair vendors or Prime <br /> Shine maintenance department to perform any necessary <br /> replacement or repair. <br /> Step 4. Ensure that all equipment and machinery involved in incident has been <br /> returned to perfect operating condition. <br /> Step 5. Document incident using Emergency Information Form located in <br /> contingency plan notebook. <br /> Step 6. Inform General Manager of incident and what steps have been taken in order to <br /> prevent incident from occurring in the future. <br /> Step 6. Employee Consultation. If incident was caused by employee misconduct <br /> proceed with Prime Shine steps for consultation and behavior correction. <br /> 4.
