Kettleman Ln. Lodi, CA 95240 <br />Program: HW <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR -Notice to Comply) <br />On site to conduct a routine hazardous waste inspection. <br />11. There are dark wet oil stains and oily debris on the ground next to the drums of drained used oil <br />filters. Facilities shall be maintained and operated to minimize the possibility of a fire, explosion, or <br />release of hazardous waste to air, soil, or surface water which could threaten human health or the <br />environment. Immediately clean up the spilled oil, oil stains, and oil debris, and manage according to <br />Title 22 hazardous waste regulations. Submit a statement and supporting documentation explaining how <br />this waste was managed by 7-20-11. Minor -Notice to Comply. <br />20. The white 55 gallon poly drum of used antifreeze does not have marked the hazardous property. All <br />hazardous waste containers shall be marked with the following information: <br />1. the words "Hazardous Waste" <br />2. name and address of generator <br />3. hazardous properties <br />4. physical state <br />5. composition (contents) <br />6. accumulation start date <br />Immediately label this container and ensure that all hazardous waste containers are marked with all the <br />required information. Class II Repeat Violation. <br />23. Three 55 gallon drums of drained used oil filters did not have marked the accumulation start date. <br />Used oil filters must be stored in a closed, rainproof container, labeled as "drained used oil filters," and <br />marked with an accumulation start date. Continue to ensure that all used oil filters are managed in this <br />way. Bryan Smit marked the accumulation start date on these drums. Corrected on site. Class II Repeat <br />Violation. <br />Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br />each violation was corrected, including supporting documentation, by 7-20-11. <br />44 <br />7 <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE ($122). <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />EHD Inspector: Received By: Title: W\1 � �, Q�� <br />EHD 23-02-003 Rev 08/10/10 CONTINUATION FORM <br />
