REPORT PREPARATION <br /> Upon completion of field work and receipt of final laboratory analysis, a report will be prepared <br /> presenting the findings of the investigation. The report will include a description of the work <br /> performed andresults of the sampling and analysis . Conclusions, applicable recommendations, maps <br /> and cross-sections will be included in the report. The report will be in a format acceptable to the <br /> SJCEHD andthe Regional Board andbe reviewed and signedby a California Professional Geologist. <br /> 3 .0. FIELD PROCEDURES <br /> All field procedures will be overseen by an AGE representative under the supervision ofa California <br /> Professional Geologist. Procedures for permitting and pre-field work activity, the advancement of <br /> the soil borings, media injection, ground water sampling, equipment decontamination and sample <br /> shipment are outlined below. <br /> 3 . 1 . PERMITTING AND PRE-FIELD WORK PREPARATIONS <br /> A notice of intent (NOI) to comply with the Waste Discharge Requirements of Regional Board <br /> General Order No. R5-2008-0149 for In-situ Groundwater Remediation at Sites with Volatile <br /> Organic Compounds, Nitrogen Compounds, Perchlorate, Pesticides, Semi-volatile Compounds <br /> and/or Petroleum Hydrocarbons will be submitted upon SJCEHD approval of this Work Plan. <br /> Additionally, applicable boring permits will be obtained from SJCEHD and registration in the U. S . <br /> EPA Underground Injection Control Program will be completed. <br /> A Site Health and Safety Plan (SHSP) will be prepared in accordance with Occupational Safety and <br /> Health Guidance Manual forHazardous Waste Site Activities (National histitute for Occupational <br /> Safety and Health Administration) and federal (29 CFR 1910. 120) and state (Title 8, Cal. Code Reg., <br /> Section 5192) regulations. The SHSP will cover: <br /> Health and safety officer and key personnel, and personnel requirements andresponsibilities; <br /> Advanced CeoEnvironmental, Inc. <br />
