n • y Mr Nuel Henderson, Jr, SJCEHD June 27,2005 Former Apache Plastics Facility, Stockton,CA Project No 2005-2050-01 Well Installation and Destruction Letter Report Page 3 locking cap was placed on the top of the well casing Well details and DWR well completion reports for each monitoring well are included in Appendix A Well Destruction Well MW-12 was pressure grouted with neat cement on March 30, 2004 The traffic rated vault box covering well MW-12 was removed, and the upper 3 feet of the well casing was overdrilled using an 8-inch diameter hollow stem auger The DWR well destruction notice for MW-12 is included in Appendix A Monitoring Well Development and Redevelopment Stratus oversaw Woodward Drilling Company redevelop well MW-8 on March 30, 2004 Well MW-8 was re-developed by surging and bailing Approximately 15 feet of sediment were removed from the well casing during re-development activities Stratus developed wells MW-15 through MW-18 on April 15, 2004 by surging with a plastic bailer, followed by groundwater pumping Stratus removed approximately 3 to 10 well casing volumes during well development activities Well MW-17 pumped dry during development activities Field data sheets documenting the development activities are provided in Appendix B Monitoring Well Sampling Wells MW-15 through MW-18 were incorporated into the ongoing monitoring program for the site during the second quarter 2004 Four sampling events have been completed to date Prior to sampling, approximately 3 well casing volumes were purged from each well Samples were collected from each well using a new plastic bailer The groundwater was subsequently transferred to laboratory supplied glass vials (voas) and stored in an ice-chilled cooler Strict chain-of-custody procedures were followed from the time the samples were collected until the time the samples were relinquished to the laboratory Waste Management Soil cuttings were stockpiled on, and covered with, plastic sheeting and stored onsite pending disposal Rinseate water generated during drilling was placed in DOT-approved 55-gallon steel drums and stored on-site pending disposal Integrated Wastestream Management of Milpitas, California, transported the soil cuttwg5 grid drummed wastewater to licensed facilities for disposal Stratus transported the other purged groundwater generated during the well development and sampling events to a licensed facility for disposal K\Apache Plastics\Reports\Apache-well-install-destruct-letter report doe
