Annual training is not being conducted for the aboveground petroleum storage program and <br />containers. Aboveground petroleum storage training was not documented. At a minimum, oil -handling <br />personnel must be trained in the operation and maintenance of equipment to prevent discharges, <br />discharge procedure protocols, applicable pollution control laws, rules & regulations, general facility <br />operations, and the contents of the facility's SPCC Plan. Annual briefings must be conducted to assure <br />adequate understanding of the SPCC Plan and highlight and describe any known discharges or failures, <br />malfunctioning components, and any recently developed precautionary measures. Immediately conduct <br />necessary training for all oil handling personnel. Keep training documentation in facility's SPCC Plan <br />binder to be available for inspector upon review. Submit how to ensure an annual training will be <br />conducted and how training logs are kept. <br />517. Facility aboveground tanks do not have written inspection logs. It is unclear who, the SPCC Plan <br />contractor or facility employee, is responsible for conducting inspection and how often. Inspections and <br />testing shall be conducted on all aboveground liquid petroleum containers larger than 55 gallons and <br />including all 55 gallon drums . Records of these inspections and tests shall be signed by the <br />appropriate supervisor or inspector and kept on site with the SPCC Plan for a period of three years. <br />Immediately begin necessary testing and inspections for all APSA regulated containers and maintain on <br />site with the SPCC Plan. Submit a written explanation who will be doing the inspections/ testing on <br />aboveground tank and how often. Explain how written inspection logs are kept. <br />Submit to SJC EHD within 30 days (07/07/14): <br />1. one copy of the completed "Return to Compliance Certification" form <br />2. a written corrective action statement for each of the above violations <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />TH FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />EHD Ins ect <br />Received <br />Title: <br />V <br />EHD 23-02-003 Rev 04119/12 V CONTINUATION FORM <br />
