Contact the local fire department and the San Joaquin Valley Air Pollution Control District <br /> for applicable permits. <br /> 2. Any changes shall be submitted to EHD for approval prior to commencing work. All new <br /> submittals supercede the previous ones. <br /> 3. If the underground storage tank and associated piping are not installed by July 1, 2003 then <br /> the new monitoring system requirements and construction requirements under Assembly Bill <br /> 2481 must be satisfied. An addendum to this permit must be submitted and approval must be <br /> received prior to commencing work. <br /> 4. Contractor training records for the APT and Smith Fiberglass piping must be submitted prior <br /> to commencing work. <br /> 5. Both primary and secondary pipes shall bear a UL label. <br /> 6. All pipe fittings and connections shall bear a UL label. <br /> 7. All monitoring systems shall be listed on the LG113-16. <br /> 8. All testing shall be in compliance with manufacturer's instruction, section 2635 and section <br /> 2636 of California Code of Regulations(CCR). Note:APT piping requires a 1-hour pressure test. <br /> 9. Automatic pump shutdown must satisfy CCR 2636(g). <br /> 10. All piping must slope to a containment sump with an audible/visual alarm. <br /> 11. All equipment in contact with backfill must be corrosion protected. <br /> 12. Overfill prevention must satisfy CCR 2635(b)(2). The audible/visual overfill alarm must be <br /> mounted in the sight/hearing range of the delivery driver. <br /> 13. An accurate and drawn-to scale-as-built plan must be submitted to EHD prior to the final <br /> inspection. <br /> 14. A complete monitoring and response plan must be submitted prior to the final inspection. <br /> 15. All lines and tanks shall be tested for tightness, in accordance to manufacturer's guidelines <br /> by a licensed tank tester and results received prior to the final inspection. <br /> 16.
