Financial responsibility documents have not been submitted to the EHD. Current financial <br /> responsibility documents are required to be submitted annually. Immediately submit a copy of the <br /> financial responsibility documents to the EHD. A current copy of financial responsibility documents has <br /> been submitted to the EHD at the time of inspection. CORRECTED. <br /> 305. The 87-octane piping sump sensor failed to activate an audible and visual alarm when tested to <br /> ensure continuous monitoring of the tank system. All monitoring equipment shall be maintained to <br /> activate an audible and visual alarm or stop the flow of product at the dispenser when it detects a leak. <br /> The technician onsite cleaned and successfully retested the aforementioned sensor during the <br /> inspection. CORRECTED. <br /> 302 & 308. Jumper hoses were found to be connected (or Schrader valves not removed) on secondary <br /> lines in all UDCs (under dispenser containments). Any potential release from the primary containment <br /> into the secondary containment in the diesel, 87-octane and 91-octane piping does not flow into a <br /> collection sump. All secondary containment systems shall be sloped so that all releases will flow to a <br /> collection sump located at the low point of the underground piping. Immediately contact a properly <br /> licensed, trained, and certified contractor to ensure that any releases in the aforementioned piping flows <br /> into the appropriate UDCs. Submit a copy of work order/receipt to demonstrate that these violations <br /> were corrected by March 23, 2014. <br /> 311. The 87 and 91-octane line leak detectors failed to detect a leak when tested. All line leak detectors <br /> shall be capable of detecting a 3-gallon per hour leak at 10 psi. The leak detectors were replaced and <br /> retested during the inspection. A "UST Retrofit Verification with Inspector Already Onsite' form has been <br /> completed and provided to the operator and contractor.
