Hazardous waste generators shall retain <br /> copies of all manifests signed off by the disposal facility and all receipts used in a consolidated manifesting procedure o <br /> site for three years and have them readily available for review. Immediately locate a copy of all manifests and receipts 1 <br /> the last three years, maintain them on site, and submit copies to the EHD. <br /> 113 HSC 25160.2(b)(4) Consolidated manifest receipts failed to contain complete information. <br /> The following consolidated manifest receipts for American Valley on these date were missing the correct EPA ID numbe <br /> 1/15/13, 11/26/13, 8/7/14 and 1/27/15, The copies had the generator EPA ID number listed as CAL00281661, when the <br /> generator EPA ID number is CAL000360778. All copies of each receipt shall contain all of the following information: <br /> -The name, address, identification number, contact person, and telephone number of the generator, and the signatui <br /> of the generator or the generator's representative, <br /> - The date of the shipment, <br /> -The manifest number; <br /> -The volume or quantity of each waste stream received, its California and RCRA waste codes, the waste stream typ <br /> listed in subdivision (c), and its proper shipping description, including the hazardous class and United Nations/North <br /> America (UN/NA) identification number, if applicable, <br /> -The name, address, and identification number of the authorized facility to which the hazardous waste will be <br /> transported; <br /> -The transporter's name, address, and identification number; <br /> -The driver's signature, <br /> -A statement, signed by the generator, certifying that the generator has established a program to reduce the volume <br /> or quantity and toxicity of the hazardous waste to the degree, as determined by the generator, to be economically <br /> practicable. <br /> Ensure that all future receipts for hazardous waste shipments using a consolidated manifesting procedure contain all <br /> required information. <br /> 301 CCR 66262.34(d)(2) Facility not maintained to minimize the release of a hazardous waste. <br /> Dark stains were found on the soil behind the used oil tank. Facilities shall be maintained and operated to minimize the <br /> possibility of a fire, explosion, or release of hazardous waste to air, soil, or surface water which could threaten human <br /> health or the environment. Immediately clean the dark stain on the soil, and manage according to Title 22 hazardous <br /> waste regulations. Submit a statement and supporting documentation explaining how this waste was managed. <br /> 401 CCR 66262.34(d)(2) Failed to store hazardous waste in a container that is in good condition. <br /> Observed a 55-gallon plastic drum of antifreeze with a hole in the lid.
