Copies of used antifreeze disposal records for 2014 were not found on site. Hazardous waste generators shall retain copies of all manifests signed off by the disposal facility and all receipts used in a consolidated manifesting procedure o site for three years and have them readily available for review. Immediately locate a copy of all manifests and receipts 1 the last three years, maintain them on site, and submit copies to the EHD. 113 HSC 25160.2(b)(4) Consolidated manifest receipts failed to contain complete information. The following consolidated manifest receipts for American Valley on these date were missing the correct EPA ID numbe 1/15/13, 11/26/13, 8/7/14 and 1/27/15, The copies had the generator EPA ID number listed as CAL00281661, when the generator EPA ID number is CAL000360778. All copies of each receipt shall contain all of the following information: -The name, address, identification number, contact person, and telephone number of the generator, and the signatui of the generator or the generator's representative, - The date of the shipment, -The manifest number; -The volume or quantity of each waste stream received, its California and RCRA waste codes, the waste stream typ listed in subdivision (c), and its proper shipping description, including the hazardous class and United Nations/North America (UN/NA) identification number, if applicable, -The name, address, and identification number of the authorized facility to which the hazardous waste will be transported; -The transporter's name, address, and identification number; -The driver's signature, -A statement, signed by the generator, certifying that the generator has established a program to reduce the volume or quantity and toxicity of the hazardous waste to the degree, as determined by the generator, to be economically practicable. Ensure that all future receipts for hazardous waste shipments using a consolidated manifesting procedure contain all required information. 301 CCR 66262.34(d)(2) Facility not maintained to minimize the release of a hazardous waste. Dark stains were found on the soil behind the used oil tank. Facilities shall be maintained and operated to minimize the possibility of a fire, explosion, or release of hazardous waste to air, soil, or surface water which could threaten human health or the environment. Immediately clean the dark stain on the soil, and manage according to Title 22 hazardous waste regulations. Submit a statement and supporting documentation explaining how this waste was managed. 401 CCR 66262.34(d)(2) Failed to store hazardous waste in a container that is in good condition. Observed a 55-gallon plastic drum of antifreeze with a hole in the lid. Hazardous waste shall be stored in a container th are in good condition. Immediately transfer all hazardous wastes into containers that are in good condition or have hazardous wastes properly disposed of by a licensed hazardous waste hauler under a proper manifest.
