An in-depth review of these documents, completed between the time periods of <br /> 71/2003 to 12/31/2003, indicates errors or omissions. These errors and omissions can be considered <br /> violations of California law; however,because this is a new program,we wanted to advise your company <br /> of these types of errors now, in order to avoid any future enforcement actions. <br /> Listed below are the enclosed manifest/log numbers and the type of errors noted: <br /> Manifest Number Type of Error <br /> M1-1104575 Invalid Manifest Number, Missing/Unreadable Facility Tire Program ID <br /> M1-1104270 Missing Pickup/Delivery <br /> M5-1121006 Multiple Pickup/Delivery <br /> M2-1104156 Missing License Plate State, Invalid Log Number, Missing Load Type <br /> M1-1203130 Missing Load Date <br /> M2-1178373 Impossible Load Date <br /> Missing Load Type, Missing/Unreadable Facility Tire Program ID, Missing Facility <br /> M9-1104138 Site Suffix <br /> M5-1104555 Invalid Facility Tire Program ID <br /> M3-1178374 Invalid Facility Site Suffix <br /> M6-1203077 Invalid Decal Number <br /> By giving you this review,we hope to call attention to deficiencies that you need to correct in future <br /> manifests and trip logs. Please make sure that these errors/omissions will be corrected in the future by <br /> completely and accurately filling out the hauler portion of the Manifest(Part I.) and Tire Trip Log forms. <br /> Further, as a hauler,you must ensure that the Generator or End-Use Facility accurately completes Part H <br /> of the Manifest form.
